Photo Permission Form

This letter is to both inform you and request permission that your picture as a student may be published on the Cooper RiverYacht Club’s website, promotional brochures or news releases. Student images may occasionally be published on the Cooper River Yacht Club Website (http://cryc.clubexpress.com), promotional brochures or news releases to promote student activities and celebrate student achievements.  However, there are potential dangers associated with posting personally identifiable information on a web site because global access to the Internet means that the Cooper River Yacht Club cannot control who may view the website.   No personal information about your child, such as home address or telephone number will be published. 

Since this program has several funding sources and sponsors who require a final report with photographs, it is necessary for you to sign this form prior to enrolling in the paddling or sailing classes.  

I/WE GRANT permission for any photograph of this student to be published on the Cooper River Yacht Club’s website, reports, promotional brochures or news releases.

In addition, I agree to release and hold harmless the Cooper River Yacht Club, its Board of Governors, agents, officers, contractors volunteers, and employees from and against any and all claims, demands, actions, complaints, suits or other forms of liability that shall arrive out of or by reason of, or be caused by the use of my child's picture on the Cooper River Yacht Club’s website, promotional brochures, reports, or news releases.
